Add subsection for the :entry-point option

Hugo Ishimaru requested to merge hishimaru/asdf:man-fix into master

  1. Since the BNF of defsystem grammar lacked the :entry-point option, I added a simple subsection for it. And then I put some @anchors in this section for convenience.

  2. I reclassified two error classes to the type index.
