Print radix marker by default in disassemblies and adjust note-column for x86
Describe the bug
disassemble
by default prints all numbers in hex, but does not include a radix marker. This can be very confusing if the output is 10
because it's easy to forget that that is 16 and not 10.
Also, some instructions for x86 are quite long and the note-column is too low so that the notes don't line up neatly. This should be adjusted.
